Hi TC! The Department of Early Education (EEC) requires super detailed information about immunizations. Is there a way a form can be created for THE FAMILIES or a doctor to self-report all the data in a way that I can then export all the data and send the numbers to EEC? The one offered in TC now looks like the person doing the entry is ME...not ideal for my limited admin time. Thanks for considering!
