Injury Reports - in daily tracking I would like the option to have an injury report we could fill out if a child gets hurt. I would want this form to be able to be customized by the school. At a minimum the form should have date and time of injury, name of injured child, type and circumstance of the injury, staff present, treatment, date and time parents were notified, method of notification, future action to prevent recurrence of injury, a place for two staff members to sign and a parent (if necessary). I would like this form to be able to be edited and returned to later in the day when signatures are obtained and then able to be locked in the chart.
